LCSW, Behavioral Health Clinic - Care Mgmt - Full-Time,Days

Position Summary: 

As a member of the Care Team, the behavioral health clinical social worker is a core member of the collaborative care team. This includes the patient’s medical provider and psychiatric consultant, as well as the larger primary care or medical team. The behavioral health clinical social worker is responsible for supporting and coordinating the mental and physical health care of patients on an assigned patient caseload with the patient’s medical provider, and, when appropriate, other mental health providers. In addition, the behavioral health clinical social worker must be able to assist the patient in a sensitive and supportive manner, while acting as an advocate on behalf of the patient.

Essential Functions and Responsibilities of the Job:

• Demonstrated ability to collaborate and communicate effectively in a team setting.
• Ability to maintain effective and professional relationships with patients and other members of the care team.
• Working knowledge of differential diagnosis of common mental health and/or substance abuse disorders, when appropriate.
• Ability to screen for common mental health and/or substance abuse disorders.
• Effectively engage patients in a therapeutic relationship, when appropriate and thru assessment and treatment planning for common mental health and/or substance use disorders.
• Utilize evidence-based psychosocial treatments and brief behavioral interventions for common mental health disorders, when appropriate (e.g., motivational interviewing, problem-solving treatment, behavioral activation and/or counseling techniques).
• Basic knowledge of psychopharmacology for common mental health disorders that is within the appropriate scope of practice.
• Contributes effectively to the overall function and organization of the Medical Foundation provider clinics and centralized Medical Management services.
• Monitor and implement all regulatory changes impacting care.
• Adhere to all company policies, procedures and directives.
• Interact in a positive, professional and constructive manner
